    Abstract

    Current tuberculosis control strategies are failing to contain the epidemic in HIV prevalent settings. A detailed understanding of where, when and how Mycobacterium tuberculosis (Mtb) transmission is occurring in communities with a high HIV/tuberculosis burden is urgently required in order to effectively implement interventions to break the transmission cycle. The highest transmission potential is in the household of untreated smear positive pulmonary TB patients. This study will be based in an HIV-prevalent rural community in Malawi as part of a long-term population-based study of tuberculosis and will complement other ongoing studies examining community transmission (eg NHSRC #944). The specific objectives are to identify risk factors for household transmission and to assess strain variation and investigate genomic determinants of Mtb transmissibility. Households of smear positive TB patients in Karonga district will be visited and screened for infection and active disease. Isolates from the patient will have mycobacterial DNA extracted and subjected to strain identification and whole genome sequencing as part of broader epidemiological studies of TB (NHSRC # 424).

    Setting: whole of Karonga district
    Study population: all children aged 10 years and under and all adults aged 18 years and over who are have been resident in the household of a symptomatic smear positive TB case in Karonga district. This includes pregnant women and those aged over 65 years.
