    Effects of Acute Illness on Contact patterns in Africa was a study that was conducted in Karonga District of Northern Malawi. It was implemented from 16th of December 2016 to 15th December 2017. The purpose of the study was to assess how acute illness affects contact patterns in an African setting in order to improve estimation of the spread of infections. We completed recruitment on 11 Nov 2017. We are at a very preliminary stage of analysis. We recruited 618 participants; 369 patients and 249 caregivers. The Study was led by Mr Albert Dube as principal investigator, Dr Steffen Geis, Professors Mia Crampin, and Professor Judith Glyn as co-investigators

    Abstract

    There are no data on changing contact patterns when ill in Africa, yet they could profoundly affect transmission of infections. We recorded named contacts (face-to-face conversations or touch) and opportunities for casual contact in a rural population in northern Malawi in three 24-hour periods: when attending clinic for an acute illness, the following day, and 2 weeks later, when well. Contact patterns for caregivers were also collected for participants aged <18 years. We interviewed 384 patients and 257 caregivers. Patients had an average of 16.8, 15.4 and 15.6 contacts on the three visits. Results for caregivers were similar. The day of the clinic visit the number of adult contacts and the proportion using taxis and minibuses were increased for all ages. Compared to the day after the clinic visit, participants seen on the same day of the week when well had a mean of 0.4 (95% CI -0.4, 1.2) extra contacts, with an increase of 0.8 (0.2, 1.3) for indoor contacts, 0.8 (0.0, 1.5) for contacts of =10 minutes, and 0.9 (0.2, 1.7) for contacts involving touch. Children tended to have more child contacts and fewer adult contacts when well, and adults tended to have more adult and fewer child contacts when well, but all differences were small (< 1 contact/day). When well participants were more likely to go to church, market and school, visit other houses and use public transport, than they were on the day after the clinic visit. Opportunities for casual contact were affected by illness more than contact with named individuals. Clinic visiting increased named and casual contacts, and being ill at home decreased such contacts. Illness increased dis-associative age mixing. These data should help refine models of infection spread.

    This study collected data on a number of areas. In addition to the demographics such as age, sex and residency; socio-economic data, illness the patient was suffering from, who the guardian or caregiver was, travelling history of both the care giver and the patient in the past 7 days prior to interview, whether they came into contact with animals, information on where they had been 24 hours prior to the interview date were collected. Both the patient and the caregiver were asked to keep a dairy and document the contacts they have made since they visited the clinic. Contact information that was collected included; initials of the contact, sex, age, meeting place, whether the meeting was in or out door, whether touching was involved and how frequent the patient or caregiver came into contact with the contact.

    Sampling Procedure

    The study was conducted within a demographic surveillance area in rural northern Malawi. Our previous pilot work showed that keeping a diary and being interviewed about contacts in the previous 24 hours are both acceptable to the population, but that both fail to include all contacts when done independently. We therefore combined the methods, using the diary as an aide memoire for the interview, and instructing interviewers to probe carefully for contacts. Contacts were defined as people with whom the participant had face-to-face conversations or skin contact, not simply people they passed and greeted.
    Project staff based at the clinic recruited participants from among those attending with symptoms suggesting acute infectious disease (e.g. fever, acute respiratory infections, diarrhoea, vomiting). Following written consent an interviewer explained to the patient and their care-givers how to keep a diary of contacts.
