    Abstract

    The primary purpose of this study was to determine the impact of pneumococcal conjugate vaccine (PCV) on invasive pneumococcal disease (IPD) following this vaccine's introduction into the national immunization program in Malawi. Additionally, the study aimed to document changes in circulating invasive pneumococcal serotypes andstrengthen support for the continued procurement of vaccine locally. Furthermore, this evaluation aimed to provide important information for other African countries considering a similar vaccine introduction strategy. The work consisted of three parts:

    1. Active surveillance for invasive pneumococcal disease
    2. Case-control studies of vaccine-effectiveness
    3. Adverse events surveillance

    Active, sentinel pre- and post-vaccine surveillance for IPD and serotypes was used for this study to assess the impact of vaccination on hospitalised IPD and on distribution of invasive pneumococcal serotypes. All children with clinical or radiological pneumonia who presented to Chilumba Rural Hospital (CRH) in Karonga District and who met the standard case definition were eligible for recruitment. All attendances at the facility and the proportion due to IPD and pneumonia were recorded.

    To assess vaccine effectiveness a case-control design was used. Case-patients were children age-eligible to receive the vaccine and were identified by active, laboratory surveillance. Controls will be children living in the same neighbourhood of case-patients. Information on vaccination status, schedules and variables potentially affecting vaccine performance (e.g. anthropometry, sociodemographic indicators) were collected and a request for an HIV test made or consent sought to use existing HIV-infection status data. Interpreting vaccine effectiveness data in conjunction with vaccination coverage rates was also to provide indirect estimates of vaccine impact on pneumococcal burden in Malawi.
